If you or a loved one have been diagnosed with mild, moderate, or severe dementia, you may be eligible for support through the federal government’s new program, Guiding an Improved Dementia Experience (GUIDE).
Program Highlights
- Services are covered under Medicare at no additional cost to the patient and caregiver.
- Comprehensive assessments to understand the specific challenges and strengths of each individual and caregiver. A personalized care plan is created for the person living with dementia and adapts as needs evolve.
- Care coordination with health care providers. You do not need to change your doctor to participate in the program.
- Access to a trained Care Navigator available 24/7 to answer questions, coordinate medical appointments, review care plans, and connect individuals and caregivers to community resources and support.
- Family caregiver training and education on how to best care for a person living with dementia. Caregivers may qualify for a yearly allowance of up to $2,563 for respite services.
- Ongoing support with regular check-ins to help families navigate the complexities of dementia care with confidence.
Eligibility
A person is eligible for the GUIDE program if they:
- Have been diagnosed with Alzheimer’s disease or another form of dementia
- Have Medicare Parts A and B as their primary health insurance
Not Eligible If
You are not eligible if you are enrolled in:
- Medicare Advantage (including Special Needs Plans or PACE programs)
- Hospice
- Program of All-Inclusive Care for the Elderly (PACE)
- Or if you are a long-term nursing home resident
